Pregnancy Gingivitis: What Every Expectant Mother Should Know

On at least three separate occasions, I can remember having the privilege of breaking the news of a young woman’s pregnancy from my side of the dentist’s chair. I use the word “privilege” with my tongue planted firmly in my cheek, however. After all, the diagnosis was only determined because of the patient’s tender, red, swollen gums. Ouch!

Talk about taking the good news with the bad . . .

Known as pregnancy gingivitis, this unpleasant condition is often assumed to be one of the natural consequences of the hormonal changes often found in pregnant women. The hormones involved, estrogen and progesterone, are secreted in progressively greater concentrations throughout most of pregnancy. While this reaction is necessary for preparing a woman’s body for the journey ahead, the resulting flood of hormones results in a variety of effects.

Hormones tell the kidneys to retain water in order to build blood volume to have enough to nourish the placenta. Thus a pregnant woman has 40% more fluid in her body. As a result, this increases the amount of fluid in all the cells in the body, including the gum tissues, which causes them to become “puffy.” Between the time of conception and the seventh month of pregnancy, hormones will triple in quantity, and then remain at that heightened level until delivery.

Meanwhile, as if she doesn’t have enough to contend with, the pregnant patient with hormones out of whack is swollen and tender and sometimes cannot do an effective job with her oral hygiene.

I am pleased to tell you that with above standard oral hygiene, this condition can be almost entirely avoided. Remember, it is the bacteria, not the hormones, which cause the gingivitis. Despite the fact that the statistics reveal 50-75% of all pregnant women develop pregnancy gingivitis, that doesn’t have to mean you. If you are reading this book after the fact, you can be assured that this condition is reversible with proper home care, techniques, tools, and medicines.

Dr. Thomas Rams, professor and chairman of the Department of Periodontology at Temple University School of Dentistry in Philadelphia, says, “There are hormonal shifts in pregnancy that reduce the gingival tissue resistance to infection, and there is also an increased risk that the hormones circulating will help promote the growth of certain bacteria and plaque, which causes inflammation.”

Rams also says, “In periodontitis, bacterial plaque infection not only causes inflammation of gingival tissues like gingivitis, but also progressively destroys connective tissue fibers and surrounding bone anchoring teeth to the jaws, resulting in the loss of teeth.”

Knowing how to minimize the bacterial biofilm in your mouth, and especially along the gum line, is the key factor in preventing pregnancy gingivitis and the escalating symptoms that often continue during pregnancy. However, a woman needs to be taught before she gets pregnant how to eliminate bacteria on a daily basis.

Because gums can be painful, puffy, and bleeding during pregnancy, the tendency is to avoid touching them. However, it is imperative that you practice good oral hygiene during pregnancy to avoid both tooth decay and gum infections. Pregnancy, with its three-fold increase in hormones, may exaggerate the body’s normal response to dental plaque.

It need not increase your risk of getting gum disease . . .

Although relatively rare, the body’s response to inflammation may produce another offset of the “joy” of childbirth: a pregnancy “tumor” (also called pyogenic granuloma). This “tumor” may develop on the gums in response to the irritating bacteria that collects there. Rest assured, however, that these growths of extra tissue are usually painless, and totally benign. However, if they get large these “pregnancy tumors” can become annoying and should be treated. Meticulous oral hygiene is the answer to avoiding the inflammation associated with these growths, and it is comforting to know that they usually subside after childbirth.

Fascinating Factoid: “Occasionally chewing sugarless gum is helpful to increase the amount of saliva in your mouth, without contributing to the risk of gingivitis. It may also be helpful in averting the nausea so common in the first three months of pregnancy.”

Brushing, flossing, and irrigating daily (all done gently but deliberately) are important in preventing gingivitis, during pregnancy and anytime in your life. In addition, it is important to pay attention to eating more healthfully for yourself and your baby, by substituting fruits, cheese, nuts, and vegetables for those sugary foods you might have enjoyed before becoming pregnant. If your gums are tender and brushing is painful, try ice, or several helpful over the counter products that will soothe painful gums. These include:

*Anbesol liquid

*Orajel Mouth-Aid

Pregnancy brings in joy with the ever-increasing waistline, pains and aches. The body undergoes a tremendous change due to shifting of hormones. The skin changes, there are stretch marks and dark patches on the facial skin. Weight gain results in sleeping troubles, poor digestion, frequent urination, constipation etc. These are just a few symptoms of pregnancy. Tender breasts: Wear a soft, comfortable maternity or nursing bra with extra support. Apply heavy moisturizing cream to cracked nipples after washing with water only.

Dizziness: Expanding uterus puts pressure on blood vessels, plus the nausea and vomiting can make one feel dizzy or light headed. To overcome this problem, maintain pregnancy health with frequent but small meals. Avoid standing or sitting in one position for long, lay down on your left side to relax, stand up slowly and do not let your body get over heated.

Hemorrhoids: They too are caused due to increasing pressure on the rectal veins and constipation. To prevent hemorrhoids drink lots of fluids, take fiber rich food and do not strain the bowel while evacuating.

Leg cramps: As the body weight increases and change in metabolization takes place, the pressure on legs and feet increases. Eat low-fat calcium rich food, go for short walks, stretch and flex leg and foot muscles, use heating pads to ease the leg cramps.

Swelling: A significant weight gain during pregnancy can result in swelling and puffiness. It could be a sign of high blood pressure or toxemia, consult the doctor immediately. To avoid swelling and to maintain a good pregnancy health, avoid salty foods and caffeine. Rest with your feet elevated and drink at least eight glasses of water to keep your body hydrated.

Teeth and gum problems: Maintain good oral hygiene by brushing your teeth regularly; avoid eating sweets and take high calcium diet. Oral bacteria due to gum disease can harm the fetus and result in either low-birth weight baby or premature delivery.

Similarly take all natural preventive measures to combat the various other changes in the body due to the pregnancy. What is Paracetemol

Paracetemol is the common mispelling of Paracetamol. Paracetamol is the most popular anti-inflammatory drug on the web. In this article from now on we will use the commonly used misspelling.

Paracetemol is an NSAID. NSAIDs are non-steroidal anti-inflammatory agents/analgesics (NSAIAs). The most prominent member of this group of drugs is Ibuprofen (Advil and Motrin). I will not give more information on the chemistry of this painkiller as it would be complex and unlikely to mean anything to our web audience.

Paracetemol is thought to be safe for short term use for pain and fever during pregnancy. The only thing to think about is if you\’re taking tablets and cold remedies, you could end up taking an awful lot.

Parecetemol is sused in preganancy. On the web one user reported; “I suffered dreadful headaches in the first trimester and didn’t want to take anything in case it harmed the fetus. I kept putting it off and stuck headache for 3 days before giving in. I later found out that I could have been using this drug. (Do always check with your doctor/medical advisor though before doing this yourself.)

However, do avoid anti-inflammatorys such as Ibruprofen in pregnancy. Paracetemol is usually safe. Paracetemol is particularly deadly for cats as it also causes liver failure. If someone has given your cat one of these tablets seek veterinary advice promptly.

Paracetemol is toxic at very low levels (if you take eight tablets in a 24hr period) and if you react badly to caffeine, don\’t take Anadin. Unsurprisingly some people are becoming addicted to branded painkillers, which cause a “medication overuse headache”. Paracetemol is pretty much the only safe painkiller to use when you are pregnant. The most common are stomach upset, heartburn, and nausea.

Each pill pack consists of 21 pills that contain the same amount of estrogen and progestin, to be taken one per day for three weeks, followed by seven placebo pills that are taken throughout the week, in order for a woman to get her period. Yasmin is the only birth control pill to contain the synthetic progestin known as drospirenone — an alternative for women who experience adverse effects from other oral contraceptives.

Alesse tablets are one of the most effective methods used worldwide for birth control (99% effective when taken as directed). Alesse contains natural hormones that stop the ovary from releasing an egg and prevent a man’s sperm from entering the uterus. So with Alesse you can put your mind at ease and enjoy intimate moments with your partner.

The pills are taken one pill every day until you finish an entire pack. It is suitable that taking the pill with a regular activity that you do at the same time every day, like eating a meal or brushing your teeth.

But you should consult with your doctor before start taking Birth Control pills as many women may not be able to take the pill because of the risk of serious health problems.

Piles – Home Remedies for Piles

Piles is a very common ailment. Women have higher incidence than men. The reason behind it is not clearly known. It is more commonly seen in adolescent age group and aged persons who all seem to be more tensed.

In medical terminology, piles is known as hemorrhoids. Piles refers to a condition in which the veins around the anus or lower rectum are swollen and inflamed. It is the dilatation (varicosity) of the veins of anus. It appears as bluish coloured swellings due to stagnated blood. Piles arise due to the increased pressure of internal or external veins around the anal canal.

Piles can be both internal and external. Internal hemorrhoids, unless they are severe, cannot be seen or felt. There is however, a discharge of dark blood. External hemorrhoids can be seen around the outside of the anus, and there is a lot of pain but not much bleeding. In some cases, the veins burst and this results in what is known as bleeding piles.

Symptoms –

Symptoms of piles vary, and depend on the type and place of origin. Piles in its initial stage is most commonly found to be without any symptoms.

However, the most common symptoms include -

• Intense pain at the time of passing stool.

• A pain around the anus and lower rectum, which can be quite severe.

• Frequent ineffectual urge for stool without passing stool or without satisfaction of complete evacuation.

• Itching in the rectal area.

• There may be bleeding. This will be bright red blood, not usually mixed in with the motion.

• There is often a feeling of something bulging, or hanging down from the anus.

• If a haemorrhoid at the outside of the anus gets a blood clot, it leads to tender lump.

• Anemia due to profuse bleeding.

• Wind formation in the stomach.

• Sitting becomes difficult due to pain.

If left untreated, piles can lead to complications including, infection, prolapse, necrosis and gangrene. Painful bleeding usually indicates startup of complications.

Causes -

Haemorrhoids are very common. It is more common in people, who regularly follow a low fibre, and high processed diet, such as processed flour, polished rice, bread etc. The main contributory cause is anything that raises the pressure in the abdomen, which leads the blood vessels to swell and become engorged.

Thus, the main cause of piles include -

- People chronically suffering from constipation.

- Even diarrhoea can cause piles. The strain for passing the stool is the main reason for the development of piles.

- After or during pregnancy, as the baby may press on the main blood vessel that returns blood to the heart.

- Overweight people.

- Physical strain – even coughing for long periods can cause the anal veins to strain.

- General weakness of the tissues of the body.

- Low fiber diet

- Age

- Hereditary

- Anal intercourse

- Prolonged periods of sitting or standing.

- Strenuous work

- Mental tension

Piles Treatment –

It is usually best to treat piles with the least treatment possible, as even after the most extensive treatments, there is a possibility that they may still return.

Usually, piles becomes better with increased water intake, a high fibre dietary regimen and other measures to correct the constipation. Care for blood loss should be taken to avoid anaemia.

Many times, they settle on their own over a matter of days without any treatment.

Haemorrhoidectomy refers to the removal of the pile by surgery. In this advanced laser treatment, the surgery reduces the pain and blood loss. However, in course of time, there is a possibility of the recurrence of piles even after surgery.

Over the counter drugs, include cream or suppositories. These soothe itching and pain, and cause the swelling and bleeding to diminish.

If the pain is very severe, cold compresses with ice can be helpful.

Homeopathic treatment, under an expert is also an effective way of dealing with this problem.

Prevention –

Haemorrhoids are very common, and may occur even after surgical removal. However, one may follow certain preventive steps to avoid recurrence –

- Avoid becoming overweight, and lose weight if you are.

- Eat a high fibre diet.

- Exercise regularly.

- Take rest and avoid long walks if you are suffering form piles.

- Drink plenty of water and follow regular bowel habits to avoid constipation.

- Use Indian style toilets since the squatting position is better for natural defecation

- If required, pass stool twice a day to avoid constipation.

- Don’t strain to evacuate hard faecal matter.

- Take easily digestible, simple homemade food. Avoid spices, chilli, very hot or cold food, and alcohol.

- Take foods such as green vegetables, ginger, onion, garlic, beetroot, bitter gourd, which helps in healing piles.

- Avoid eating potato, and the yellow variety of pumpkin.

- Take plenty of high fibre fruits especially papaya, mango, lime, jamun, figs, etc.

- Avoid sedentary habits, alcohol, smoking and unnecessary drugs like painkillers.

Some useful home remedies for piles –

- Take 1 table spoon of roasted black cumin seeds and mix with 1 tablespoon of unroasted black cumin seeds and powder both together. Take 1/2 a teaspoon of this mixture with a glass of water once a day.

- Drink a glass of buttermilk with 1/4 tsp carom seed powder (ajwain) and a pinch of rock salt.

- The jamun fruit should be taken with salt every morning for two or three months during its season.

- Soak 2-3 dried figs in a glass of cold water overnight. Have it first thing in the morning on an empty stomach. Have them for 2-3 weeks for good results. It is effective in the treatment for ordinary as well as bleeding piles.

- Mix 1 teaspoon fresh mint leaves juice with 1 teaspoon lemon juice & 1 tablespoon honey. Take 3 times a day for relief from piles.

- Take ½ cup radish juice in the morning and night for a period of 1 month.

- Boil 1 ripe banana in 1 cup of milk, mash well. Take 3-4 times a day.

- Extract 3 teaspoon of bitter gourd juice, and mix in 1 glass of buttermilk, and drink on an empty stomach every morning, for a month.
